2022 is a “carry over” year for the Wellness Program Rewards of $0 PCP copays and reduced Specialist co-pays. This means that if you completed the online health survey and selected/confirmed your PCP through your medical plan’s microsite as well as completed at least one recommended screening in 2021, wellness program rewards will continue through December 31, 2022.
If you did not complete these activities, you still can! Complete these activities anytime in 2022 and you will enjoy the rewards through 12/31/2022. View or download the Wellness Plan Requirements document [PDF] for more details.

Transform Diabetes Care® will be transitioning to a new, no-cost meter on January 1, 2022 – BioTel Care cellular-enabled meter.
Starting January 1, 2022, the Livongo meter you’re currently using won’t be part of the Transform Diabetes Care® program (you don’t need to return it). To make sure you continue to receive personalized care without interruption, you’ll need to order a new BioTel Care blood glucose meter and supplies before the date above. If you are currently enrolled, you should receive a letter by mid-October detailing the transition and how to obtain your BioTel meter. If you are not currently enrolled, you can do so by calling BioTel.
